Dear Parent/Guardian:
As we come to the close of the school year, we would like to remind you to please pick up any medication that has been signed into the Jay High School clinic for your child. Medication should be picked up by closing on the last day of the
school year. No medication may be kept or stored over the summer break. Unclaimed medication will be disposed of after school is dismissed on the last day of the school year. To plan for the upcoming school year, remember that if you would like your child to carry and self-administer epinephrine, pancreatic enzymes, or asthma inhalants please obtain a new physician authorization with signature and date on the “Santa Rosa County School District Dispersion of Medication Form”. Updated physician authorization for students to carry and/or self-administer diabetes medication and/or supplies will also be required for the new or upcoming new school year.
